This IMO dangerous goods form meets the requirements of SOLAS 74, chapter VII, regulation 4; MARPOL 73/78, Annex III, regulation 4 and the IMDG Code, Chapter 5.4. The IMO dangerous goods form requires specific information regarding commodity, hazard classification, shipper, receiver, point of origin to final destination, and much more. The IMDG Code form may consist of more than one page, provided pages are consecutively numbered.

The snap-out forms are available in 8 1/2" x 11", 4-part and packed 100 sets/package. The pin-feed forms are available in 8 1/2" x 11", 4-part and packed fan-folded 500 sets/box.
