The Multimodal Dangerous Goods form is the declaration recommended by the International Maritime Organization (IMO). The International Maritime Organization requests this documentation identify specific commodities being shipped, the hazard classification, point of origin to the final destination, and more. The Multimodal Dangerous Goods form is provided to expedite the movement of hazmat shipments between modes, e.g. truck to ship.

The Multimodal Dangerous Goods form comes in laser, snap-out or pin-feed formats. The laser forms are 8 1/2" x 11", 1-part, single sheet and packed 100 sheets/package. The snap-out forms are available in 8 1/2" x 11", 4-part and packed 100 sets/package. The pin-feed forms are available in 8 1/2" x 11", 4-part and packed fan-folded 500 sets/box.
