The Canada Customs Invoice form (CCI) is required by Canadian Customs officials to verify the value, quantity, and nature of the shipment of goods. The Canada Customs form is required on all export shipments passing through customs en route to Canada. The CCI form can be prepared either by the exporter, importer or their agents and must include 1) name and address of vendor, 2) name and address of consignee, 3) shipping date, 4) country of origin, 5) quantity of goods and 6) value of goods.

The snap-out forms are available in an 8 1/2" x 11", 5-part, bilingual format and packed 100 forms/package. The laser forms are available in an 8 1/2" x 11", 1-part, single sheet, bilingual format and packed 100 sheets/package. The CCI form is bilingual, and printed in English and French. The Canada Customs forms meets current standards and specifications of the National Revenue's Custom & Excise Department.
