The Straight Bill of Lading form is used when shipping standard commodities, hazardous materials or combinations thereof. The non-hazardous materials shipping form, hazardous materials shipping form, or combination shipping forms are available in snap-out, pin-feed and laser formats. The Straight Bill of Lading form has been revised to include both basic description sequences. The snap-out forms are available in 8 1/2" x 11" and 8 1/2" x 8 1/2" sizes, 4-part and 3-part carbonless, packed 100 forms/package. The pin-feed forms are available in 8 1/2" x 11" and 8 1/2" x 8 1/2" sizes, 4-part carbonless, packed fan-folded 500 forms/box. The laser forms are 8 1/2" x 11", single sheet, packed 100 sheets/package.
