New - Uniform Hazardous Waste Manifest, 5-Part Pin-Feed Form

The new 5-part Uniform Hazardous Waste Manifest, EPA Form 8700-22 (Rev. 12-17), is a hazmat shipping form prepared by all generators who transport, or offer for transport, hazardous waste for off-site treatment, recycling, storage, or disposal. This new 5 part form is required by the EPA beginning June 30, 2018, but is not allowed for use prior to this date. When completed, it contains information on the type and quantity of the waste being transported, instructions for handling the waste, and signature lines for all parties involved in the disposal process. The 5-part manifest is required when using paper manifests associated with the EPA e-Manifest system, and also meets DOT requirements. This pin-feed form measures 8 1/2 x 11 in. and is sold in fan-folded packs of 500.
